Job Description:
- Coverage Needed
- December 1, 2024 - March 31, 2025
- Schedule
- 3 days in clinic
- Other days will be doing documentation, home visits, or skilled
nursing facility visits
- Call is not required (their is an option for provider to help
with call if they wish)
- Call is nights 5 pm - 8 am, weekends Saturday 8 am - Monday 8
am
- Call is beeper (after hours nurse usually answers first or the
operations director gets a call and then calls the provider)
- Clinical Details
- 4 Patients Per Shift
- 90% work is clinic based, 10% is home visits and nursing home
visits
- Provider will be working independently, provider will not be
100% on their own but the day to day work in the clinic will be
independent.
- Requirements
- Board Certified Required
- Active CA License Required
- BLS Required
- Experience with long term care and geriatric medicine
- Minimum of 2 years + experience
- Support Staff
- 2 RN's
- 1 MA
- 1 Office Staff
- EMR
